How to fill the HRA

Click on HRA Program (Located on the lower left corner of the Conxent Dashboard)

Read through the message informing agents about a program that could maximize their potential.

We have a special program where agents participating in the HRA program for open enrollment 2025 will be compensated $25 for each successfully completed HRA. Members must be enrolled for 90 days, and payment will be made to the agent on the 120th day. After the submission date, any members who fall out within 90 days will not count, and the agent will not be paid the $25 for those cases.

To proceed, the agent needs to click the acknowledgment toggle and send the HRA agreement to themselves, just as with previous forms. There are options to send the agreement in English or Spanish via email or SMS. After the agent has signed, they can start guiding the consumer through the health risk assessment.

The HRA program dashboard will only display consumers who have both the consent and eligibility forms signed. Under the HRA status, it will show "Pending" if the HRA has not yet been attempted.

To begin the HRA, go to the "Action" button and click on "HRA." Here, you will see a breakdown of the enrollment year, ZIP code, and other personal and demographic information.

After confirming the personal and demographic details, press "Save." The HRA consent email will be sent to the consumer. Once the consumer agrees and signs the consent, the HRA will populate.

If, for any reason, the HRA questions do not appear, press the refresh icon in the upper right corner. Once the questions are visible, review them with the consumer and confirm that they are ready to sign off on their answers.

A benefit of the form is the option to add another member if assisting a family. In the upper right corner, you'll see an "Add Member" option. From there, specify their relationship to the primary consumer, enter their personal and demographic information.

Once the consumer confirms they're ready to sign off on the HRA, they will receive an email with all the questions and their answers. They will also have the option to download a PDF copy. After the consumer signs, return to the HRA program dashboard, where you'll see that the HRA status has changed to "Complete."
